In an effort to reduce mailing costs, save time/resources, and increase efficiency, 4-H National Conference has developed electronic methods for Delegation Coordinators to submit youth delegate Medical Forms and Code of Conduct Forms. We encourage you to use the electronic methods for this year, but, will allow the mailing of forms to our registration center if that is easier. Starting in 2013 forms will only be accepted electronically.

Option 1: Upload Individual Youth Delegate Forms

Scan your delegate forms and upload them delegate by delegate.
You will have the option to upload a scanned Medical Form and Code of Conduct separately or as one file. Select whichever format works best for you.

Option 2: Upload Youth Delegation Forms in Bulk

Scan you entire delegations form into one file and submit them as one package online.
You may upload scanned Medical Forms and Codes of Conduct separately or as one large file. Select whichever format works best for you.

Option 3: Fax Your Forms to (202) 318-9119

Our fax service will digitize your forms and email them to us.
You may fax them individually or you can fax one large set. Whichever works best for you.
